The Indian Health Service (IHS) is currently undertaking a strategic realignment of its organizational structure, representing the agency's first comprehensive review in more than two decades. According to IHS, the initiative is designed to strengthen patient-centered care, advance Tribal self-determination, modernize enterprise services, and create a more agile organization that can better meet the needs of American Indian and Alaska Native communities.
As one of the nation's largest federal healthcare systems, IHS serves American Indian and Alaska Native communities through a combination of federally operated facilities and Tribal healthcare programs. Over the past two decades, the healthcare landscape has changed significantly. Today, Tribes directly operate approximately 65 percent of the IHS budget through the Indian Self-Determination and Education Assistance Act, making continued organizational modernization increasingly important.
Periods of organizational change require thoughtful planning, open communication, and trusted partnerships. While agencies work to strengthen long-term operations, healthcare delivery must continue without interruption. Patients still need access to quality care, providers need dependable support, and healthcare facilities must remain fully operational throughout periods of transition.
